Get Started with Playworks

Here are the steps to initiate service with Playworks:

Contact our Intake team by clicking on the Contact Our Intake Team button below.
Within 48 hours, we will send you a link to a secure web form (using the email address you provide) and some potential times for an intake call.
After you've completed the Intake Form, we will call you to to learn more about your child, answer any questions you may have, and discuss scheduling options.

Here are a couple of other forms that we'll discuss later. You don't need these for initial intake.

Consent for Services Form
Please print, sign, and bring the Consent for Services Form to our first scheduled meeting.
Parents can also fill this out in the office.

Credit Card Authorization Form
Please print this Credit Card Authorization form, fill out the hard copy, and give the form to your therapist at the first scheduled meeting. Playworks Speech, Language & Occupational Therapy accepts all major credit cards. We will charge your card automatically, unless you request to pay by cash or check and payment is made at the time of service. If a credit card transaction is unsuccessful and payment has not been received, regrettably we cannot commit to any additional service.
